The Complex Medical Management service at Providence Care Hospital meets the needs of patients age 16 or older who are medically stable and have medically complex needs. Patients have multiple co-morbidities and/or disabilities typically requiring skilled nursing and technology-based care that cannot be provided by community programs/services.
Complex Medical Management is offered to inpatients as a means to avoid further loss of function; increase activity tolerance; and slow the rate of or avoid further loss of function to achieve transition home or to the next level of care.
Admission to the program is determined on a case-by-case basis for patients who meet eligibility criteria. Patients must demonstrate a need and an ability to participate and benefit from low to moderate intensity therapy, including Physiotherapy, Occupational Therapy and/or Speech Language Pathology. Twenty-four nursing care is required. Individualized goal-based programs are developed and supported by a team of health professionals.
