Providence Care conducts regular surveys of our patients/clients/residents, families of long-term care residents, as well as our staff and physicians. Results of the surveys conducted in 2010 are available HERE.

Quality Improvement Plan (St. Mary’s of the Lake Hospital, Mental Health Services)

The 2012/13 Quality Improvement Plan (QIP), is one tool that Providence Care is using to help document and review current performance in a variety of areas. With this plan, Providence Care is able to see our targeted areas for improvement clearly, and to chart our progress. READ MORE

Providence Care’s website reports the rate and number of cases of Clostridium difficile (C. difficile), Methicillin Resistant Staphylococcus aureus (MRSA) and Vancomycin Resistant Enterococcus (VRE) at our two hospital sites: St. Mary’s of the Lake and Mental Health Services. READ MORE

Accountability Agreements

Providence Care has three accountability agreements with the South East LHIN, which maintain current service levels and establish funding levels to allow Providence Care to continue providing high quality clinical programs and services. MORE INFORMATION

Board of Directors - Meeting Minutes

Executive Contracts

The compensation structure of Providence Care’s senior managers is comprised of several components. The performance and compensation of the President & CEO is reviewed each year by the Performance Management & Compensation Subcommittee, a subcommittee of Providence Care’s Board of Directors. Providence Care’s President & CEO annually reviews the performance and compensation of each member of the senior leadership team. This yearly evaluation is tied to the compensation for all members of the senior team.  MORE INFORMATION - HERE
