The Outreach Program offers a neurobehavioural approach that involves cognitive, emotional, behavioural, physical and pre-vocational rehabilitation. The program is for adults who have had a moderate to severe brain injury who are able and willing to identify and work towards their own goals with assistance from staff.
Potential participants are assessed to determine the suitability of the program and to make recommendations for goal achievement. An individualized service plan is then developed and reviewed as needed. The service rests on roles as goals, learning by experience in real-life contexts and the use of personal and environmental supports to enable participation.
Participants are usually involved in both individual and group activities at our offices, home and community. Each person has their own schedule. Emphasis is placed on daily living skills, social integration, preparing for productive activities such as working or going to school, improving self-awareness, controlling emotions and behaviour, and maintaining satisfying relationships.
