The Attendant Care Outreach Program (ACOP) provides assistance of daily living to individuals with long-term physical disabilities in their homes.Clients are required to direct their own care and should fully understand the nature of their disability. A commitment to excellence in care is provided as part of Providence Care’s Mission to treat each person with dignity and compassion.
Referrals are accepted from hospitals, physicians, social or community health agencies, as well as from friends, family, or the client themselves. Based on the individual's needs and the hours available, clients can receive up to a maximum of 21 hours of pre-scheduled attendant services per week. Services are available seven days a week, from 6 a.m. to 12 a.m. midnight.
The Attendant Care Outreach Program office is located at Providence Manor, at 275 Sydenham Street, Kingston, Ontario. The program serves Kingston, Frontenac, Lennox and Addington counties. ACOP also operates the Endymion Supportive Living Unit.
Eligibility:
Open to anyone aged 16 and older with a permanent physical disability which prevents him/her from carrying out activities of daily living. Clients must be willing and able to direct their own services and remain unattended outside of prescheduled times.
