The Geriatric Outreach Assessment Service is a community resource for seniors who are coping with multiple health problems. It is also a resource for families and family doctors.
The Outreach Service specializes in the assessment/review of each client’s issues, needs and concerns; geriatrician review of medical issues and medications; evaluation of the need for and the potential for gains from rehabilitation.
The Outreach Service works with clients, families and other caregivers, as well as family physicians, Community Care Access Centre case managers and other healthcare professionals.
How does the Outreach Assessment Service work?
Once a referral is received* and determined appropriate, one of the team members will calls to arrange an in-home visit to gather information. A family member, caregiver, etc. is welcome to be present during the visit.
With permission, we contact other people in a client’s “circle of care”
Our team (physician, nurse, occupational therapist, physiotherapist and social worker) then works together to develop recommendations for the client and his or her family doctor (a copy of the Outreach final report is sent to the family doctor) aimed at helping with current issues and preventing further problems.
What might some of the outcomes of this geriatric assessment be?
- Further tests to add to our understanding of health issue(s)
- Ways to promote more independence
- A change to medications
- Linkage with other community-based services
- A short period of rehabilitation such as the Day Hospital (outpatient) or Geriatric Inpatient Unit
- Attendance at a geriatric clinic
* Please note that anyone can make a referral, however, the family physician must be included as he/she plays a key role in follow-up. Seniors without a family physician are welcome to contact us to discuss their options regarding geriatric assessment.
