The Multidisciplinary Clinic is a resource for seniors who are coping with multiple health problems, as well as their families, and family doctors. Clients with multiple medical, functional and social issues are seen in a clinic setting by a physician who specializes in the care of the elderly. Additional assessment and input in other disciplines (nursing, occupational therapy, physical therapy, and social work) is available.

Anyone can make a referral, however the family physician must be included as he/she plays a key role in follow-up.

The Multidisciplinary Clinic specializes in Assessing each client's issues, needs, concerns and rehabilitation potential, as well as the review of geriatric medical issues and/or medications. The Multidisciplinary Clinic also provides linkage to other Specialized Geriatrics and/or community services as appropriate and initiation of treatment whenever possible.
