The Day Hospital provides short-term outpatient assessment and rehabilitation for seniors with multiple health needs who are likely to benefit from seeing a multidisciplinary team of health care professionals.
The Day Hospital specializes in the assessment/review of each client’s issues, needs and concerns by a team of health care professionals and management of multiple health issues and rehabilitation.
Like the Outreach Service, the Day Hospital works with clients, families and other caregivers, as well as family physicians, Community Care Access Centre case managers and other health care providers.
How does the Day Hospital work?
Once accepted to Day Hospital, the Care Coordinator calls to make arrangements for three review/assessment visits. Each visit entails meeting with different team members. These team members include a nurse, occupational therapist, physician, and physiotherapist, social worker and support staff. We are also a teaching facility and may have health care students assisting in assessment and treatment. The three assessment visits will be on three separate mornings over a two-week period. After these visits, during which the client will have met with all of the team members, they will develop a plan for how they can help or make other recommendations. The Care Coordinator will call the client to talk about the plan. If it includes treatment at the Day Hospital, the client can decide at that time if he or she would like to attend.
Treatment programs are offered in the afternoons twice weekly. The client may see the pharmacist and recreation therapist, as well as the team members mentioned above.
Clients should continue to visit their family doctor as per usual while attending and following discharge from Day Hospital.
