The Complex Continuing Care (CCC) is a comprehensive inpatient service for adults who require 24-hour nursing care and who can benefit from a multidisciplinary program. Individuals are now being referred closer to the end stage of the disease process and often have multiple needs. Providence Care’s complex continuing care services are located at the St. Mary's of the Lake Hospital site, in Kingston.
There are a limited number of respite beds at St. Mary's of the Lake to provide relief to families who are caring for a disabled or elderly relative at home, who meets the admission criteria for complex continuing care. Except for emergencies, the bed must be pre-booked. Admission cannot exceed thirty-one days.
As part of the continuum of care, the Complex Continuing Care program provides a coordinated, inter-disciplinary team assessment and treatment for patients whose medical conditions are stabilizing and who require, at this time, on-going complex care within a hospital setting. The program serves individuals who require technical assistance and close monitoring and/or 24-hour nursing care and supervision appropriate to a complex continuing care hospital.
Referrals to CCC are accepted from acute care hospitals, family physicians, long term care facilities and other health care providers, i.e. CCAC.
