The Community Outreach Team provides intensive case management services for persons with severe mental illness living in the community. The team also assists clients who are ready to 'step down' from the more extensive support provided by the Assertive Community Treatment (ACT) teams. The Community Outreach Team is part of the Providence Care Schizophrenia Rehabilitation Service and is based at Community Support Services, 525 Montreal Street, Kingston.
- Ongoing treatment, rehabilitation and support to help clients reach their goals for community living
- Weekly medication clinics
- Assistance with living arrangements and finding employment
- Shared care with family physicians
- Referrals to other service providers
- Liaison with families, physicians, and all providers involved in the client's care.

Referrals:
Clients can self-refer or be referred by family, mental health workers, hospitals, family physicians and community agencies.
